New Heights Clinic is a free clinic that serves people with medical and dental needs who are unable to obtain care because they have no medical or dental insurance and have real difficulty paying for care. Patients are seen by appointment only.

- Narcotic Prescriptions
- Prenatal Care
- Urgent Needs
- TB Tests
- Physicals for: Workman's Comp, Department of Transportation, Employment, School, SSI, or Disability

New Patient Appointments
New Heights Clinic is limited on the number of patients it can serve.
- Call and leave a message on Thursday morning at 8:00am, 360.694.0355.
- Appointments are made in order of messages received.
- Appointments must be confirmed by noon the day before or they will need to reschedule.
- Appointments scheduled for Monday need to be confirmed by Thursday noon.

Your First Visit
- Confirm your appointment by noon the day before it is scheduled.
- Arrive 15 minutes early.
- Sign in.
- Complete new patient packet (to the right).
- Bring medication bottles of each prescription.

- Simple root canals (front teeth)
- Dentures, crowns, and cleanings are not available

Appointments
New Heights Clinic is limited on the number of patients it can serve.
- Patients seen by appointment only.
- A ppointments are made from telephone messages left every other Thursday at 5:00 PM. The number is 360-694-0096.
- The limited number of appointments available are first call, first served.
- Your call will be returned the next day, if we have an appointment for you.
- Appointments are scheduled no more than two weeks out.
- All appointments must be confirmed by noon the day before or they may be given to another waiting patient.
- We are unable to guarantee a request for a specific dentist, as the dentist volunteer times are variable.
The Clinic is staffed by volunteers and is not able to provide emergency care. In the event of a dental emergency you will need to make other arrangements.
First Visit
- Arrive 15 minutes before appointment time.
- Sign in with name and birth date.
- Complete the New Patient Packet.

- Enrollment & Message Confidentiality form.
Your information will be reviewed and x-ray(s) will be taken, as needed. The dentist will check your mouth and confer with you as to what can be done to help in the time that is available. The dental team will provide the care that has been decided upon.
A follow up appointment will not be made unless the dentist requests it for a specific reason. Additional appointments are made by calling in to try for another time.

Monday (occasional): 6:30pm - 9:30pm
Wednesday (occasional): 5:30pm - 8:30pm
